​Liability & Medical Release: Elite Youth is not responsible for any injury (or loss of property) to any person while observing, taking class, attending camp, participating in special events, or in any other way involved on the premises. In consideration of my child's participation, I hereby release and covenant not to sue Elite Youth or any of its owners, employees, coaches or agents collectively referred to as "EY" from any and all present and future claims, property damage, personal injury or wrongful death arising as a result of my child engaging in or receiving instruction. I further agree to indemnify and hold harmless Elite Youth and all others listed for any and all claims arising as a result of my child engaging in or receiving instruction in activities at Elite Youth. Additionally, I do hereby grant permission for my child to participate in leadership and/or activities with Elite Youth, owner Stephanie Kalkbrenner, their staff or assistants. I also grant permission for, and encourage, any necessary emergency medical treatment that may be required due to injury or medical need. I acknowledge and accept that I am accountable for any medical expenses incurred as a result of such treatment, I understand and agree that I will be responsible for all medical bills.

​Media Waiver: I understand that from time to time, Elite Youth, and its staff, employees, and representatives records and photographs its participants during live sessions, online sessions and special events and all other events associated with Elite Youth (collectively, the "Events"). I hereby consent and grant permission for Elite Youth, in any and all formats or media, whether now known or hereinafter to be discovered or developed, to: (i) record, use and edit (a) my or my child’s name, voice-signature, photograph, image, likeness, distinctive appearances, gestures, mannerisms, and any and all biographical information submitted as part of my participation in the Events, and (b) any performances in connection with the Events (collectively, the "Rights"); (ii) reproduce, create derivative works based upon, distribute, publicly perform, publicly display, webcast and sell the recording in which Elite Youth makes use of the Rights, in whole or in part (the "Recordings"); and (iii) to use the Recordings in advertising, marketing, or promoting Elite Youth or its events and programs. All permissions granted by me are irrevocable and effective throughout the world, and apply to Elite Youth and its assigns, contractor, sub-licensees, distributors, media partners, successors and agents.
